  • Percentage breakdown of volume by subscription / membership term.
  • Cancellation Policy must be provided and be clearly visible to consumers.
  • Please provide us with a copy of any Welcome Package or form emails that get sent out to the
    customer
    (include an example of a recurring billing email notification if applicable).
  • Provide Name Fulfilment Organizations (if applicable) that fulfil the discounts and copy of agreement
  • Describe the transaction chain of events from the initial phone/web contact with the customer to the
    delivery of the merchandise / completion of the service.
  • 3 months most recent processing statements
