  • Legit Script or VIPPS certified
  • Executive summary on business, owner, and key personnel.
  • List of all URL’s that will be accepting credit cards.
  • Pharmacy licenses
  • Information about the pharmacy merchant’s source of, and supply chain for, the drugs it receives.
  • Current Balance Sheet and P&L statements
  • Two years of Business Tax Returns.
  • Three months most recent processing statements
  • Three months most recent bank statements
